office 24 hours in advance and reschedule your appointment to a more

Office Visit:
Please make sure that you bring your picture ID and your insurance
cards, current medication list when you visit our office for your
appointment. Please note that we do not accept personal checks.
Please fill out Medication list and bring with you. If you like your
PCP/physician to send us your records, please fill out "Medical Records
Release" and fax it to us. If you want our office to send your records to
your physician, please fill out "Medical Records Release II" and fax it to
us.

We participate in many major plans with various insurance companies
such as Aetna, BCBS, CIGNA, Humana, Medicare, United Healthcare
etc . Since the insurance plans change quite often, we suggest that you
call your insurance company and check with them about our
participation in their plans. We also accept patients who pay cash.
HIPAA Compliance:
We are compliant with HIPAA (our compliance) regulations.
