appointment or drop by our office. Our front desk staff will be

If you are unable to make it to the appointment, please call our
office 24 hours in advance and request a cancellation and your
appointment can be rescheduled to a more suitable time of your choice.
We urge you to keep your appointment. It is in your best interest.
Office Visit:
Please make sure that you bring your picture ID and your
insurance cards, current medication list when you visit our office for your
appointment. We do not accept personal checks.
We require the following documents to be filled out completely and
signed by you. We recommend that you complete these forms and
bring with you when you come for the office visit.

Insurance Participation:
We participate in many major plans with various insurance
companies such as Aetna, Blue Cross Blue Shield, CIGNA, Humana,
Medicare, United Healthcare etc . Since the insurance companies offer
a large number of plans that change quite often, we suggest that you
call your insurance company customer service line and check with them
about our participation in their plans.
We also accept patients with no insurance plans but will pay cash.

We are compliant with HIPAA (our compliance) regulations.
